Effect of a rice bran supplement before dietary meal on fasting and post-
prandial glucose and cholesterol levels of type 2 diabetic patients in Thai-
land

Rationale: A recent dietary intervention study by Songchitsomboon S et al suggests that a low-glycemic
index (LGI) dietary advice emphasized on consuming brown-rice variety 313-19-1 or Sinlek (GI = 58) which
contains high dietary fiber and many types of antioxidant, is beneficial for plasma glucose and lipid control in
type 2 diabetes mellitus (T2DM). Therefore, rice bran of brown rice Sinlek may have potential application as
functional food.
Objective: The purpose of this study was to determine the effect of supplementation of high nutritive
value rice bran Sinlek as rice bran pills (consumed 3 times/d, total 30 g / d) before meals on changes in blood
glucose, 2-hr postprandial plasma glucose, and lipid profile.
Method: Fourteen type 2 diabetic patients with fasting plasma glucose (FPG) ≥ 126 mg / dL, consumed
an American Diabetes Association (ADA) diet as baseline diet for two weeks, then were randomly allocated to
participate in a cross-over trial with two six-week diets separated by a 6-week washout period. The study
intervention consisted of rice bran pills plus ADA diet and only the ADA diet. During both study periods,
subjects were asked to maintain a consistent level of physical activity throughout the study. Fasting plasma, 2-
hr postprandial glucose (PPG) by oral glucose tolerance test (OGTT) and fasting lipid profile were determined at
the beginning and the end of each 6-week period. Blood pressure, anthropometric measurements and dietary
assessment were also performed at the start, middle and end of each period by the same nutritionist.
Results: A statistical analysis indicated that there were no significant differences in the intake of energy
and macronutrients including carbohydrate, protein, fat and dietary cholesterol. Results showed that after 6 wks
of rice bran pill supplement before meals plus ADA diet, mean FPG, PPG at 30 and 60 mins, and fasting serum
total cholesterol levels were decreased significantly from baseline, whereas no marked change was observed
with the ADA diet alone. One explanation for the improvement of blood glucose and cholesterol after 6 weeks
of rice bran pill supplement of Sinlek is the increased dietary fiber from rice bran pills.
Conclusions: It seems likely that supplementation of rice bran pill from Sinlek before meals has beneficial
effects for blood glucose and total cholesterol control in type 2 diabetic patients with FPG ≥ 126 mg / dL.
